
The SFSW is a city-wide program in San Francisco. Our target population are individuals over 60 years of age, who are underserved by the current social services and have a mental health diagnosis. This program is intended to reach some of San Francisco's most underserved senior citizens.
FSA's Senior Full Service Wellness Program provides the following services:
Clients can meet with one of our counselors wherever they live: apartments, streets, in a community location, or in our office.
Once an individual agrees to participate in the program, the first focus of the team is the client's basic needs for shelter, food, clothing, and medical care. Any pressing health needs will be treated through the Curry Center Clinic.
Each client is assigned a primary clinical case manager who monitors the activities of the team and has primary responsibility to work with the participant in developing his/her own individual treatment plan.
Our service is driven by client-identified goals with a focus on wellness and recovery. Treatment plans also include initiating applications for food stamps, general assistance, MediCal, and other public and private assistance programs.
