Family Service Agency of San Francisco utilizes the most comprehensive dual diagnosis system available for seniors, emphasizing persistent, patient, engagement with even the most wary seniors.

We follow the staffing guidelines found in the SAMHSA National Registry of Effective and Promising Practices (NREPP) for Assertive Community Treatment.

Utilizing a consumer and professional degree service team, we have a broad range of services at the clients‘ disposal, including physical health care, mental health care, medication management, substance abuse treatment, employment assistance, post-employment support, benefits assistance and advocacy, and peer support integration into a single service team.

The program has a program director, master‘s-level clinical case managers, peer aides, nurse practitioners, and a psychiatrist.

2. Initial Admission Criteria Checklist

Once completed, fax the forms to the number listed on the form.
